Description: | During a boat drill on an operating unit a fatal incident resulted in the death of one person and the hospitalisation of 3 others.
The boat was at deck level with seven persons onboard when the officer in charge of the winch observed that the push buttons did not function. The officer aborted the launching and instructed the electrician to heave the boat up again by manual operation, which necessitate that the various safety functions are over-ridden.
As the boat reached its top position the winch continued heaving as the limit switch function had been over-ridden. This caused the wires to snap and the boat fell 16 metres into the sea. |

Recommendations: | During drill launching and recovery, avoid having more persons in the lifeboat than are required to operate it.
In case of problems with the hoisting equipment, circumstances permitting, evacuate the lifeboat before alternative methods are used.
If it becomes necessary to over-ride the normal electrical heaving function during the drill, then it must be ensured that all personnel involved understand the risks and agree approriate precautions.
On MODUs equipped with emergency stops on the lifeboat winches the responsible personnel must be familiar with their operation. Their use should be demonstrated during drills.
The final winching prior to reaching the stowed position must always be carried out by hand.
A risk assessment should be carried out for the lifeboat launching and recovery process. |
